Get Active Questionnaire 

The Get Active Questionnaire by CSEP is a 2-page, self administered, pre-participation for exercise screening tool.

The questionnaire with its accompanying Reference Document enables you to make an informed decision whether you should seek further advice from a health care provider or qualified exercise professional before becoming more physically active.  The Get Active Questionnaire is the current CSEP-endorsed evidence-based pre-screening tool and replaces previous tools.

Completion of an evidence-based screening tool is a required element for qualified exercise professionals. While CSEP's Get Active Questionnaire is the current tool we endorse several pre-participation screening tools exist that can be used such as: The American College of Sports Medicine (ACSM)/American Heart Association (AHA) Pre-participation Questionnaire (AAPQ), the Adult Pre-Exercise Screening System (APSS) used in Australia, and the current edition of the Physical Activity Readiness Questionnaire Plus (PAR-Q+)

Para ser usado por obstetras e profissionais de Educação Física. Este instrumento foi traduzido e adaptado transculturalmente, seguindo recomendações internacionais, do original "PARmed-X for Pregnancy" (2013), por Roberta Bgeginski, MSc., F.B. Schuch, MSc. e Dr. J.G.L. Ramos, MD, Hospital de Clínicas de Porto Alegre, Universidade Federal do Rio Grande do Sul/Brasil.

The PARmed-X for Pregnancy is a 4-page guideline for exercise prescription and health screening prior to participation in a prenatal fitness class or other exercise. For use by health care providers and fitness professionals. This instrument was translated and culturally adapted, following international recommendations, based on the original "PARmed-X for Pregnancy" (2013), by Roberta Bgeginski, MSc., F.B. Schuch, MSc. and Dr. J.G.L. Ramos, MD, Hospital de Clínicas de Porto Alegre, Universidade Federal do Rio Grande do Sul/Brasil.
